My only thought is that there might have been corruption in _darcs/current
prior to recording this patch. Is this a repository that can be publically
viewed? If so, making a tarball of it available is probably the easiest way
of figuring out what happened. I'm getting rather experienced at
performing darcs forensics.
You could also try (in a copy of the repo) unrecording the patch that was
just recorded and then running darcs check on that repo. If my supposition
is right, you should see a problem there, and the precise nature of the
problem may hint at what went wrong. In a third copy of the repo, you
could unrecord the patch and then run darcs repair (which according to the
errors you're seeing should succeed) and then compare _darcs/current in the
two new directories, which might be the easiest way to see what went wrong.
On the other hand, there could have been a problem in pending, in which
case the diagnosis might be quite different. If the repo itself isn't
something you can make available, seeing even just the summary of this
patch "darcs annotate --patch centered --summary" could be helpful.
