Generally?  We yell at project maintainers that are too lazy to watch
for case-sensitivity issues.  Honestly it's easy enough to avoid
case-sensitive file names that I think most projects should avoid them
by default.

There are enough alternatives, though.  I use Windows primarily but I
certainly have several Unix boxen around for various tasks.  If I
really need to work with such a repository I can just as easily SSH
(Putty) into a unix box from my windows machine or create a quick
Virtual PC image (which Virtual PC is now free to Windows owners).

Apparently Cygwin also has some methods for handling this, but I don't
use Cygwin.
